If anemia was detected in the routine 28-week blood sample or if the womans clinician considered her hemoglobin concentration to be too low, the woman was advised to purchase and take a high-dose iron supplement (containing = 80 mg/tablet) until the end of pregnancy.
In the past decade, the amount of information linking appropriate diet and balanced nutrition to improved perinatal health status has exploded. Physicians can benefit from understanding the influence of maternal nutrition on fetal growth and development, as well as on maternal health status. Nutrition assessment and counseling should be an integral component of preconception and prenatal care: for all women.[1] Before women become pregnant, they should be advised of the profound effects nutrition may have on reproductive outcome; their current diet should be carefully reviewed, and they should be instructed about the nutritional requirements of pregnancy. If nutritional risks are identified during this process, a plan outlining a strategy for dietary modification is helpful. Several factors identify a pregnancy at nutritional risk, including adolescence, anemia, abnormal prepregnancy weight, multiple gestation, medical illness or medication that interferes with absorption of vitamins and minerals, cigarette smoking, alcohol abuse and low socioeconomic status. Common nutritional problems during pregnancy include nausea and vomiting, pica, lactose intolerance, constipation, high sodium intake, high caffeine intake, alcohol and substance abuse, and inappropriate weight gain. Current guidelines from the Institute of Medicine (IOM) at the National Academy of Sciences recommend that most women gain 11.25 to 15.75 kg (25 to 35 lb) during normal pregnancy.[2,3] Inadequate or excessive maternal weight gain has been implicated as a risk factor for many pregnancy-related problems (Table 1). The IOM recommends that target ranges be indexed to the pregravid maternal body mass index (BMI). The BMI estimates body fat. It is calculated by dividing weight (in pounds) by height (in inches) squared, and multiplying the result by 705.2. A BMI that is greater than 27 is associated with obesity and its consequences. A BMI less than 15 is considered risk for starvation. A BMI of 19.8 to 26 is normal during pregnancy.
Researchers performed a study to determine the effect of MNT on the nutrient intake of pregnant women and to determine their attitudes and beliefs concerning pregnancy and weight gain. The subjects were recruited from a population of women receiving obstetrical care at a family medicine residency center where referral for MNT was a standard part of prenatal care. Women were excluded if they had gestational diabetes, hyperemesis gravidarum, or multiple gestation. Thirty-one women, ages 16 to 37 years, participated. A dietitian took anthropometric measurements including height, weight, and wrist circumference. Ideal body weight (IBW), percentage IBW, percentage IBW for gestational age, total energy need, and target weight gain based on gestational age were calculated for all subjects. A four-page assessment questionnaire contained questions concerning demographics, financial status, common gastrointestinal problems, use of prenatal vitamin and mineral supplements, exercise habits and pregnancy knowledge and concerns. The subjects were questioned about food consumption including foods avoided, frequency of restaurant eating, and changes made in food consumption during pregnancy. A 24-hour recall was also obtained at the initial visit and after follow-up MNT sessions with dietitians. Questions regarding attitudes toward pregnancy and weight gain were also addressed. Laboratory data from the subjects medical charts was obtained including hemoglobin, hematocrit, and blood glucose levels. Subjects had two MNT sessions with dietitians during their pregnancy. It was determined that 42% of the women were concerned about excessive weight gain during their pregnancies. The low-income group in the study significantly increased their caloric intake after their first MNT session, while the other groups did not. The researchers emphasize the value of physicians, dietitians, and other health professionals stressing the importance of weight gain with pregnant women early on in the pregnancy. The dietitian can further give recommendations on how to slow or increase weight gains through evaluating an individuals diet.

The diets of pregnant adolescents are often inadequate and can have adverse effects on both the mother and fetus. Research in the United States shows diets of pregnant adolescents that are low in calories, iron, zinc, magnesium, folate, vitamins D, E, and B6, and calcium. However, little is known about the nutrient adequacy of the diets of adolescents in Guam, a United States territory located in the western North Pacific Ocean. Pregnant adolescents were recruited from Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) clinics and public schools in Guam. Pregnant adolescents, aged 14 to 20, living in Guam were included in this study. The subjects were primarily Chamorro and Filipino. Research assistants using culturally appropriate methods conducted a 24-hour dietary recall and interviewed the subjects at the start of the study. Dishes and food models were used for quantification aids. The diets were then analyzed with the Food Intake Analysis System. If an estimated average requirement (EAR) is established for a nutrient, the EAR cut point method will be used to estimate population inadequacy. However, comparisons to the recommended dietary allowance (RDA) or adequate intake (AI) can provide more limited information. Differences among trimester at program entry existed for four nutrients: thiamin, riboflavin, and phosphorus, which were all above the EAR and RDA, and iron, which was lower for those entering the study in the second trimester than in the third. Calcium intake declined with increasing age and phosphorus intake varied with age, but all groups had intakes above the EAR and RDA. The mean intake was below the EAR for folate, vitamin E, and magnesium and over 80% of the samples had intakes below this level. Iron does not have an EAR, but 63% of the adolescents had intakes below 75% of the RDA, while 78% of the sample consumed below 75% of the AI for calcium.

Physicians can benefit from understanding the influence of maternal nutrition on fetal growth and development, as well as on maternal health status. Common nutritional problems during pregnancy include nausea and vomiting, pica, lactose intolerance, constipation, high sodium intake, high caffeine intake, alcohol and substance abuse, and inappropriate weight gain. Current guidelines from the Institute of Medicine (IOM) at the National Academy of Sciences recommend that most women gain 11.25 to 15.75 kg (25 to 35 lb) during normal pregnancy.[2,3] Inadequate or excessive maternal weight gain has been implicated as a risk factor for many pregnancy-related problems (Table 1). The IOM recommends that target ranges be indexed to the pregravid maternal body mass index (BMI). The BMI estimates body fat. It is calculated by dividing weight (in pounds) by height (in inches) squared, and multiplying the result by 705.2. A BMI that is greater than 27 is associated with obesity and its consequences. A BMI less than 15 is considered risk for starvation. A BMI of 19.8 to 26 is normal during pregnancy.
Researchers performed a study to determine the effect of MNT on the nutrient intake of pregnant women and to determine their attitudes and beliefs concerning pregnancy and weight gain. The subjects were recruited from a population of women receiving obstetrical care at a family medicine residency center where referral for MNT was a standard part of prenatal care. Women were excluded if they had gestational diabetes, hyperemesis gravidarum, or multiple gestation. Thirty-one women, ages 16 to 37 years, participated. A dietitian took anthropometric measurements including height, weight, and wrist circumference. Ideal body weight (IBW), percentage IBW, percentage IBW for gestational age, total energy need, and target weight gain based on gestational age were calculated for all subjects. A four-page assessment questionnaire contained questions concerning demographics, financial status, common gastrointestinal problems, use of prenatal vitamin and mineral supplements, exercise habits and pregnancy knowledge and concerns. The subjects were questioned about food consumption including foods avoided, frequency of restaurant eating, and changes made in food consumption during pregnancy. A 24-hour recall was also obtained at the initial visit and after follow-up MNT sessions with dietitians. Questions regarding attitudes toward pregnancy and weight gain were also addressed. Laboratory data from the subjects medical charts was obtained including hemoglobin, hematocrit, and blood glucose levels. Subjects had two MNT sessions with dietitians during their pregnancy. It was determined that 42% of the women were concerned about excessive weight gain during their pregnancies. The low-income group in the study significantly increased their caloric intake after their first MNT session, while the other groups did not. The researchers emphasize the value of physicians, dietitians, and other health professionals stressing the importance of weight gain with pregnant women early on in the pregnancy. The dietitian can further give recommendations on how to slow or increase weight gains through evaluating an individuals diet.
The diets of pregnant adolescents are often inadequate and can have adverse effects on both the mother and fetus. Research in the United States shows diets of pregnant adolescents that are low in calories, iron, zinc, magnesium, folate, vitamins D, E, and B6, and calcium. However, little is known about the nutrient adequacy of the diets of adolescents in Guam, a United States territory located in the western North Pacific Ocean. Pregnant adolescents were recruited from Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) clinics and public schools in Guam. Pregnant adolescents, aged 14 to 20, living in Guam were included in this study. The subjects were primarily Chamorro and Filipino. Research assistants using culturally appropriate methods conducted a 24-hour dietary recall and interviewed the subjects at the start of the study. Dishes and food models were used for quantification aids. The diets were then analyzed with the Food Intake Analysis System. If an estimated average requirement (EAR) is established for a nutrient, the EAR cut point method will be used to estimate population inadequacy. However, comparisons to the recommended dietary allowance (RDA) or adequate intake (AI) can provide more limited information. Differences among trimester at program entry existed for four nutrients: thiamin, riboflavin, and phosphorus, which were all above the EAR and RDA, and iron, which was lower for those entering the study in the second trimester than in the third. Calcium intake declined with increasing age and phosphorus intake varied with age, but all groups had intakes above the EAR and RDA. The mean intake was below the EAR for folate, vitamin E, and magnesium and over 80% of the samples had intakes below this level. Iron does not have an EAR, but 63% of the adolescents had intakes below 75% of the RDA, while 78% of the sample consumed below 75% of the AI for calcium.

The subjects that are tested on the certification exam attest to this diversity. They are title issues; real estate finance; environmental and land use; condominiums and homeowner associations; landlord tenant; closings; sellers, buyers, brokers, and title insurers; real property litigation; taxation of entities and other tax issues; and ethics. The diets of pregnant adolescents are often inadequate and can have adverse effects on both the mother and fetus. Research in the United States shows diets of pregnant adolescents that are low in calories, iron, zinc, magnesium, folate, vitamins D, E, and B6, and calcium. However, little is known about the nutrient adequacy of the diets of adolescents in Guam, a United States territory located in the western North Pacific Ocean. Pregnant adolescents were recruited from Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) clinics and public schools in Guam. Pregnant adolescents, aged 14 to 20, living in Guam were included in this study. The subjects were primarily Chamorro and Filipino. Research assistants using culturally appropriate methods conducted a 24-hour dietary recall and interviewed the subjects at the start of the study. Dishes and food models were used for quantification aids. The diets were then analyzed with the Food Intake Analysis System. If an estimated average requirement (EAR) is established for a nutrient, the EAR cut point method will be used to estimate population inadequacy. However, comparisons to the recommended dietary allowance (RDA) or adequate intake (AI) can provide more limited information. Differences among trimester at program entry existed for four nutrients: thiamin, riboflavin, and phosphorus, which were all above the EAR and RDA, and iron, which was lower for those entering the study in the second trimester than in the third. Calcium intake declined with increasing age and phosphorus intake varied with age, but all groups had intakes above the EAR and RDA. The mean intake was below the EAR for folate, vitamin E, and magnesium and over 80% of the samples had intakes below this level. Iron does not have an EAR, but 63% of the adolescents had intakes below 75% of the RDA, while 78% of the sample consumed below 75% of the AI for calcium.

Recent research has shown that energy intakes of women during pregnancy increase very little. Apparently, women in western societies do not eat more during pregnancy because they significantly reduce their physical activity levels. This decrease in activity evidently secures the necessary energy. It is unclear, however, whether such relative reduced energy intake also secures adequate supplies of nutrients during pregnancy. * Essential Fatty Acids. The need of infants for essential fatty acids is a focus of much current research. Additional research is needed on the essential fatty acid needs of the fetus and the effects of fatty acids on growth retardation and visual function.

Women have known for decades that one-size-fits-all is a fallacy, and now a new study in the American Journal of Clinical Nutrition indicates that researchers suspect pregnant womens nutritional needs are not a generic, one-size-fits-all concept, either. The study followed healthy women from pre-conception through post-partum to see how they met the energy demands of pregnancy, and if current dietary recommendations typically given out during prenatal care were sufficient. A pregnant woman undeniably needs more energy for the growth of the fetus, placenta, uterus, and breasts, as well as for fat storage and for her metabolism to maintain all these new tissues. Thirty years ago, experts decided a woman needed an extra 250 to 300 calories a day to support all this growth. Recently, the National Academy of Sciences changed these recommendations to 150 calories in the first trimester and 350 in the second and third. Elizabeth Somer, M.A., R.D., author of Nutrition for a Healthy Pregnancy says that the key is not how much you gain as how you gain it. Weight gain in pregnancy should be a gradual process, she states. No spikes in the scale reading, but an even, steady gain, with minimal gain in the first trimester and a half a pound to a pound a week in the second and third trimesters.

WASHINGTON — Dieters got a new tool Wednesday to help them take off the extra pounds — the first government-approved nonprescription diet pill. The Food and Drug Administration said the fat-blocking weight- loss pill orlistat, which has been available by prescription, can be sold in a reduced-strength version over the counter. The new version will be sold as “alli” by GlaxoSmithKline PLC. Xenical, the prescription version, is made by Roche Holding AG. The drug is intended for people 18 and older to use along with a reduced-calorie, low-fat diet and exercise.
The city of San Diego has joined eight California counties in a lawsuit against a New Jersey diet pill maker. The action seeks a court order to halt Cytodyne Technologies and its corporate officer, Robert Chinery, from making alleged false and misleading advertising claims about the product Xenadrine, a weight-loss supplement that contains ephedra. In its advertising, Cytodyne claims Xenadrine can bring weight loss 17 times faster than diet and exercise alone. The company also claims its product increases strength and endurance. Those claims constitute unfair business practices under California law, the city and counties said.
